All referrals should be faxed to 9190 5381. Medicare rules require all following four details for it to be a valid referral;

1. The name of one of our doctors (eg. Dr Yuen et al.) at the appropriate address details 

2. A referring doctor's name

3. Their provider number

4. Their signature
